How Providers Take A Value-Based Care Approach to Chronic Disease

A report revealed that success in using a value-based care approach when supporting patients with chronic diseases may depend upon having strong care coordinators.

- The value-based care approach to chronic disease management relies on a committed clinical workforce, healthcare organizations have indicated.

Hayden Schmidt, assistant editorial researcher at Xtelligent Healthcare Media, broke down the results of an Insights report on Healthcare Strategies. The report addressed how providers are pursuing chronic disease management using a value-based care approach.

“Going into this survey, I came in with the idea that we were going to see a lot of organizations relying on their health IT as a way to manage patients at scale with their chronic diseases and tie that into their revenue cycle and value-based care payments. What we ended up finding is that it's actually a lot of reliance on people within the organization,” Schmidt explained.

The report demonstrated that even small healthcare organizations do leverage health IT in order to conduct chronic disease management but that the overarching strategies relied on a care coordinator to organize and disseminate information. This role was key to employing data well in order to improve patient care.

While the care coordinator role emerged as essential, the survey respondents also identified ways in which health IT is enhancing their value-based care strategies around chronic disease management.

“One of the respondents directly mentioned how, using data analytics and using the patient data from their EHR, they were able to identify the sort of medication management strategy that was best for their patient, and also notice lapsed prescription refills and things like that,” Schmidt mentioned.

“So when we look at what the health IT is actually doing for these organizations, one of the big things that it's helping them do is find those gaps in care and actively address them.”

Every organization will have to employ its own version of value-based chronic disease management since every organization is unique, Schmidt acknowledged. But the report revealed overarching principles that can help empower provider practices to support their patients with chronic diseases using value-based care models.
